Pinhead is split into two entities when he discovers that he was once human in Hellraiser II. One is Captain Elliot Spencer, his former self. Another is Pinhead.

While Spencer ends up in limbo, Pinhead is trapped in the Pillar of Souls. It contains the Cenobites of the Order of the Gash and the Lament Configuration.

J. P. Monroe is the owner of a nightclub named The Boiler Room. He goes inside the Pyramid Gallery and buys the Pillar of Souls. He placed it in his bedroom.

A struggling television reporter Joey stays in a hospital to cover a story. There she sees a teenage girl embedded with hooked chains. After a while, the chains pull in the air and the girl’s body is blown away.

Terri brought the young girl to the hospital. So, Joey asks Terri what happened to the girl. She replies the chains come from the puzzle box at The Boiler Room nightclub. Terri gives the puzzle box to Joey and flees away.

Joey investigates the box’s history and finds a lot of information like Pinhead, Elio Spencer, and videotapes of Kristy.

Since Terri has no place to live, Joey invites her to shit in her apartment. So Terri moves to Joey’s apartment.

Accidentally a clubgoer named Sandy goes near the Pillar of Souls and Pinhead consumes her lifeblood by the hooked chain. He slays Sandy’s skin and eats the whole body. The scene of the movie is so disturbing to watch naturally.

Pinhead emphasizes that they have both utilized Sandy for their purposes. Monroe is initially horrified by Pinhead’s suggestion but agrees to let him feed on more club members so he can be freed. In return, Pinhead promises Monroe power and other unnatural pleasures.

Joey has recurring nightmares about her presumed death in Vietnam. One night, Spencer contacts Joey. After losing faith in humanity during WWI, he sought out the forbidden pleasures promised by the puzzle box. Spencer warns her that Pinhead will wreak havoc on Earth indiscriminately for his amusement.

To defeat him, Joey must reunite Spencer’s spirit with Pinhead and return him to Hell using the puzzle box. Terri mistakenly believes Joey has abandoned her, so she returns to her ex-boyfriend Monroe.

Monroe tries to feed her to Pinhead, but she overpowers him. Rather than let her flee, Pinhead convinces her that he can turn Monroe into a demon if she feeds him, Monroe.

Free at last, Pinhead massacres the club’s patrons. When Joey hears the news reports, he goes to the club to investigate.

Joey escapes Pinhead after he orders her to bring him the box. He resurrects several of his victims as demonic Cenobites. The new Cenobites pursue Joey through the quiet streets. In a church, Joey begs the priest to help her as Cenobites attack local police. Despite lacking faith in the existence of demons, the priest is appalled by the appearance of Pinhead.

After Pinhead defiles the church and nearly kills the priest, Joey regains his attention and makes him chase after the puzzle box she holds. Joey has been captured by the Cenobites on a construction site and is preparing to be tortured. After solving the puzzle box, they are sent to Hell.

Joey is transported into limbo, where she comes face-to-face with her father, an apparition that appears to be her. In disguise as Pinhead, the apparition tells Joey to give him the puzzle box.

Pinhead ensnares her in machinery and prepares to turn her into a Cenobite. As Spencer’s spirit transitions into Pinhead, he forcibly transforms into him. Using the puzzle box as a dagger, Joey stabs Pinhead through the heart, sending him back to Hell. Joey is returned to Earth after Pinhead’s humanity is restored. She buries it in concrete at the construction site. A building with an identical interior design to the box is revealed later on the same site.
